Has your child used any prescription medication for weight loss or obesity in the last 90 days?

What is your child's sex?

Has your child started puberty? (Signs like body hair, voice changes, or growth spurts)

Does your child weigh more than 60 kg (132 lbs)?

Is your child currently 12, 13, or 14 years old?

Has your child ever had weight loss surgery or used a weight loss device?

Will your child be able to give verbal or written assent to join the study?

Will your child be undergoing a DXA body scan?

Are you able and willing to provide written consent for your child to join this study?

Has your child ever been told by a doctor that their weight is in the 95th percentile or higher for their age and sex?
